For New Patients
New patients are required to schedule an initial evaluation so we can learn more about your health history, chronic pains and struggles, and health goals. Fill out a New Patient Form and present it to us during your first visit or through email.
Once the initial evaluation has been concluded, you can now begin discussing treatment options and schedules with Dr. Lipton.

For Existing Patients
Patients we have worked with in the past can sign in to our patient portal to schedule an appointment, pay their bills, see their medical chart, and ask our doctor questions. Log in to our portal to see more features.
Insurance
Our clinic is out of network for some insurance companies. If “Personal Choice” or “Independence Blue Cross” appear anywhere on your insurance identification card, then you must pay in full for the service you are getting.
